We go the extra mile to support you in your new home
When providing SIL we are keen to hear feedback from the person we support and their support team and family. We hold regular check-in meetings and look for opportunities to continuously improve our care. This can include extra training and mentoring for our care staff, identifying ways to better support community engagement and bringing in specialist practitioners who can work on individual goals with the person we are supporting.
We develop teams based on the needs of the people we support and have experience in:
- Matching support workers by age and interests to ensure the participant has common interests and can continue engaging with activities they love with carers who understand them.
- Rapidly assembling teams to support urgent changes of living arrangements, helping people when they discharge from hospital and as they move between SDA dwellings.
- Providing blended teams with nurses and psycho-social practitioners embedded with disability carers to ensure we comprehensively respond to physical and psycho-social disability as well as co-morbid health needs.
- Providing workforce training to meet the changing needs of a participant and ensuring we are continuously improving the quality of care being received.
- Recruiting staff to meet the language and communication needs of the person we support.
- Providing ongoing recruitment to make sure the team best meets the preferences of the person receiving our support.
- Undertaking the staged reduction of support as participants develop greater independence and their support needs reduce over time.
- We are accustomed to supporting plan reviews and can provide assistance to support coordinators and guardians through review processes.
