Via Care is a registered National Disability Insurance Scheme (NDIS) Provider, that offers Homecare & Community Care services to people with a disability.

Servicing all regions of Metropolitan Melbourne, Via Care is rapidly growing and expanding its care services in all regions of Melbourne. We have specific locations in Greenvale, Roxburgh Park, Footscray, Sunshine, Pascoe Vale and Spotswood areas.

Our office-based team have been on a continuous improvement journey, and we are setting new standards in how we look after our clients and our support workers to ensure cohesion and excellent communication.

We are looking for experienced Disability Support Worker Superstars to join our growing services, working 1:1 with our participants/clients.

Our Disability Support Workers contribute to the achievement of servicing disability clients who require immediate service and ongoing roster and care.

Assisting clients in their homes and in the community around the Melbourne Suburbs. This role will suit those who are self-motivated, organised and flexible.

You will be hired directly by Via Care as an employee or subcontractor (TFN or ABN).
The Disability Support Workers responsibilities will include and not limited to:

  • Weekly ongoing scheduled attendance and service of clients, while maximising work force capacity, creating a positive client experience and adhering to NDIS compliance requirements.
  • Firm understanding of the NDIS Code of Conduct, Incident Management and Quality and Safeguards practice standards
  • Handling client issues/incident reporting to ensure they are managed in a timely and professional manner
  • Liaising with Via Care managers, and client plan nominees to gather accurate details for client Care Plans
  • Coordinating with the Operations Manager & Payroll team, for shift confirmation and adjustments
  • Understanding client NDIS Plans, goals, and Care Plans, to ensure shift tasks are met
  • Personal care, manual handling, and medication administration
  • Transport clients to appointments and community access, as per client requirements
  • Relaying important and crucial updates and alerts to the Management Team
  • Supporting a client centric culture and proactive approach
  • Contributing to the provision of relevant information and reporting
  • Contributing to process documentation and continuous improvement activities
  • Providing information to and participating in different care teams as appropriate.

We are seeking a highly driven and motivated individuals with strong verbal and written communication and relationship building skills. The right people for our team will possess strong problem-solving skills and exhibit a can-do attitude.
Experience in working with people with a disability is a must!

Requirements:

  • Must have evident experience in Disability Care and the NDIS
  • Updated First Aid & CPR
  • Valid Driver’s License
  • Current Police Check
  • Working With Children Check
  • NDIS Worker Screener Check
  • COVID-19 Vaccination Certificate
  • NDIS Module Certificate

Salary: $32.29 – $58.12 per hour
